Roles & Responsibilities
Health Care Support Workers will contribute to the delivery of holistic healthcare within Community Nursing, as delegated based on their agreed competency level, to patients and their families or carers in their homes. Ideal candidates will hold an NVQ Level 3 in Health and Social Care.
As a member of our community multi-disciplinary team, you will collaborate with District Nurses, Specialist Nurses, Therapists, and Health Care Assistants within a primary care network team to meet the needs of complex community-based patient care. This support aims to facilitate early discharge and prevent admissions to acute care facilities. A full UK driving license and access to a car for work purposes are essential.
